Our office approached you earlier and provided you with your appointment date, time, and preparation.

In order to CONFIRM YOUR APPOINTMENT, we ask you to complete the following steps :

1. Download Preparation Instruction sheet (as we instructed) and read it.
2. Download and answer the Patient Questionnaire form. Mail it back to us as soon as possible. We review your form before the appointment.
3. Fill in Confirmation Form (below) and submit it to us.
